Relapse is not a failure of treatment … relapse is a hallmark of any addiction, in fact it is a hallmark of any chronic disease, be it diabetes, or chronic hypertension, or rheumatoid arthritis. Treatment should not be discontinued because of a relapse.
Medical Weight Loss
Consumer Reports did an analysis of the effectiveness of Contrave and came to the conclusion that the weight loss from Contrave was not significant enough and that it came with high incidence of risks.

Smoking Cessation
Bupropion, commonly known as Welbutrin and Zyban, acts as a nicotinic receptor blocker, and increases the availability of Norepinephrine and Dopamine in the brain. It is an inexpensive and popular atypical antidepressant that does not cause sedation or weight gain. It is also used for smoking cessation.
Family Practice
Some providers accept insurance, but most do not accept it, because insurance coverage is patchy with no clear policy, and is restricted to approved psychiatrists, addiction specialists, or addiction programs. The majority of Suboxone prescribe-rs are not psychiatrists or addiction specialists, … most of them are internists, family practice doctors, or pain clinics.
Emergency Care
The more extreme form of withdrawal is called “Delerium Tremens”. It happens in people who have been using alcohol for a very long time, and in large quantities, who have suddenly no access to alcohol, … it takes a few days to develop. The symptoms are very dramatic; they include confusion, shaking, agitation, hallucinations, seizures, vomiting, and dehydration. If allowed to progress to this point … it will require a visit to the Emergency Room or an inpatient facility.
Electrocardiogram
Additional tests: Most programs require a CHEM-7, CBC, and an EKG. If you are an IV drug user or have additional medical problems then additional diagnostic tests are required, and charged accordingly. These tests are usually covered by your insurance policy if done through your PCP.

Pregnancy
Pregnant patients that are addicted to opioids and or other substances should discuss this with their obstetrician and seek referral to a high-risk pregnancy unit.

I trained to do neuraxial and peripheral nerve blocks, chemical neurolysis, implanting pumps for intrathecal opioid delivery, and learned medical management of chronic pain. I did not obtain board certification for pain management.
