Urinary incontinence has recently gained considerable attention in the United States. It is estimated that approximately 10 to 20 million people (10-35% of the U.S. population) are suffering from urinary incontinence. Nearly 50 percent of the institutionalized elderly are incontinent. The estimated cost of diagnosis and treatment of this group is $15 billion per year. Though these numbers are staggering, about half of incontinent patients do not alert their physician or family members of their problem. Unfortunately, most of these individuals assume nothing can be done for incontinence or feel that leakage is a normal part of aging.

Medications: Certain medications, for instance Diamox®, a medication used to treat glaucoma, increase the risk of stones because it increases calcium excretion, but inhibits citrate excretion (a chemical excreted in the urine that prevents stone formation).

In an effort to standardize terminology, the National Institutes of Health proposes dividing prostatitis into four main categories: Acute bacterial prostatitis (category I); chronic bacterial prostatitis (category II); chronic abacterial (nonbacterial) prostatitis (category III), which is subdivided into inflammatory (IIIA) and non inflammatory (IIIB) prostatitis (collectively known as ‘chronic pelvic pain syndrome’); and asymptomatic inflammatory prostatitis (category IV, detected in the absence of symptoms when prostate tissue is removed for another reason; for instance, an elevated PSA). Prostatitis is inflammation of the prostate gland.

Fortunately, urological procedures have evolved dramatically since the mid-1970s. Now minimally invasive techniques have reduced recovery times, decreased pain and scarring, and have better success rates. Mark McClure, MD, a board-certified urologist with Landmark Urology and Complementary Medicine, has actively participated in this evolution during his quarter-century of medical practice, and his many happy patients attest to the benefits of these advanced techniques. “Surgical techniques have improved because we have a better understanding of the anatomy and because new biocompatible graft materials can reproducibly restore normal pelvic support,” he explains. Dr. Daniel McRackan has the advantage of being able to offer these new products right away to his patients.
Kidney Cancer
The most common kidney cancers are renal cell carcinoma, Wilm’s tumor, transitional cell carcinoma, oncocytoma, and angiomyolipoma.
Kidney Stones
Kidney stones affect men (1 in 8 ) more often than women (1 in 20), and Caucasians more often than African Americans. According to some estimates, 25 million men and women will develop a kidney stone during their lifetime. Furthermore, after the first stone episode, the likelihood of forming another stone is 50% within five years, and 80% over a lifetime. Fortunately, preventive measures discussed below lower this risk to 10% or less.
Testicular Cancer
Testicular cancer is the most common cancer in men from age 18 to 34. Fortunately, when detected early, cancer of the testicle is one of the most easily cured types of cancer. It is crucial for testicular cancer to be diagnosed and treated at an early stage; as it may spread throughout the lymph node system into the lungs and remaining parts of the body if not detected early.

Interstitial cystitis, described as a constant irritation of the bladder that is more common in women than men and typically hard to diagnose and treat.
Urinary Tract Infection
Urinary tract infections (abbreviated ‘UTI’) account for over 7 million visits to health care providers annually and cost the US health care system 1 billion dollars annually.1 Nearly fifty percent of women will develop a urinary tract infection during their lifetime. Eighty percent of these women will subsequently develop another UTI within the ensuing 18 months. Women with recurrent UTIs typically experience two to three infections per year. Although it not always possible to prevent recurrent UTIs, the following information can help women devise a program to manage recurrent urinary tract infections.
Prostate Cancer
The most common cause of a bladder outlet obstruction is an enlarged prostate. Additional causes include can include prostate cancer, narrowing of the urethra or bladder outlet scarring from infection, injury or surgery or bladder stones. Bladder outlet obstruction may also occur as a side effect of certain medications, such as antihistamines and decongestants.

Dr. Pineda has enjoyed practicing general urology at Inland Urology Medical Group since 2001. His educational career began at the Univ. of California at Berkeley where he graduated with a Bachelors of Arts in Physiology in 1990. He completed his medical doctorate at the University of California at Davis in 1995. By 2001, he had completed 2 years of general surgery and 4 years of urology training at Loma Linda University Medical Center. During this time as a resident physician, he was the author of a book chapter, abstracts and articles on several urologic topics including urinary incontinence and minimally invasive urologic surgery. His volunteer work during this time included a medical mission to Honduras working with pediatric urologists to treat congenital urologic disorders. He has been board certified in Urology since 2003.
Minimally Invasive Surgery
Dr. Pineda treats a variety of adult and pediatric urologic disorders. He has a special interest in the treatment of kidney stones, female urology (including the treatment of urinary incontinence), benign prostate disorders and genitourinary cancers. He also has expertise in minimally invasive surgery for the treatment of kidney stones and laparoscopy (including the use of the da Vinci robotic surgical system). He is an assistant clinical professor at the Western University School of Medicine, and holds leadership roles at Pomona Valley Hospital Medical Center and Casa Colina Hospital for Rehabilitative Medicine.
Vasectomy
Vasectomy has been performed for over 100 years for a variety of reasons, however it has only routinely been used for birth control since the 1950s, when it was deemed to be a safe form of birth control. Vasectomy is a safe and effective form of birth control. Approximately 7% of married couples utilize vasectomy as a permanent form of birth control. Every year roughly 500,000 vasectomies are performed in the United States.

Throughout his academic and medical career he has been awarded many research and scholarship awards. Dr Hickerson received his training during the advent of minimally invasive and robotic surgery. This has afforded him significant experience with laparoscopy and DaVinci robotic surgery. He has a specific interest and passion for prostate cancer and is an expert in advanced robotic prostatectomies as well as specialized treatment for advanced prostate cancer.
Reconstructive Surgery
Advances in surgical techniques for reconstructing pelvic organ prolapse (POP) have given new hope to women while restoring their quality of life. Up to 50% of women will develop POP in their lifetimes. Thirty years ago, pelvic reconstructive surgery required a long period of recuperation yet only corrected the problem half of the time. Surgeons had to resort to attaching poor-quality tissue to poor-quality tissue. Not surprisingly only 10 to 20% of women with POP seek medical attention.
