Other chronic diseases: Vitamin D deficiency is also increasingly associated with multiple sclerosis and depression, while optimal levels may be helpful in maintaining normal blood pressure and keeping inflammatory markers such as C-reactive protein normal. Supplementation to optimal levels appears to help reduce the incidence of both Type I diabetes in children, and Type II diabetes in adults, and the risks for multiple sclerosis, rheumatoid arthritis and osteoarthritis.

* Thyroid dysfunction: An over- or underactive thyroid is common in the postpartum period especially in women with a family history. Though hair loss is not uncommon, when accompanied by unusual fatigue, cold or heat intolerance, palpitations, excessive weight loss or inability to lose weight, it’s a good idea to discuss the possibility of an underlying thyroid problem with your doctor. Correcting the problem with medications can be a first step in helping you feel like yourself again.

Vitamin D: this may be the single most important nutrient in preventing the flu! Since we get a lot less sun exposure (our best source of D) during winter months, supplementation may be wise even if you spend time outdoors. Dosage recommendations vary depending on your diet, immune status and blood level of D. Ever since Katie Couric underwent a colonoscopy on live television in 2000, the test, which screens for colorectal cancer, has become demystified. Dubbed the “Katie Couric Effect,” the number of colonoscopies performed in the following year increased, and the test has become more widely used. The American Cancer Society reported in April that colorectal cancer rates have dropped by 30 percent between 2000 and 2010, thanks largely to efforts to increased colorectal screening rates.

The amount of information—and misinformation—available to pregnant women is overwhelming. As a result, many mothers-to-be have a hard time trying to separate fact from fiction when it comes time to have a baby. Below, I’ll try to set the record straight regarding common pregnancy and childbirth myths.
Labor and Delivery
A small medical staff allows Riverside OB/GYN to offer a caring atmosphere and a personal treatment approach that considers each woman’s medical history, symptoms and goals. For example, our doctors counsel patients interested in a more natural pregnancy how to achieve a low intervention birth, avoid a C-section or safely undergo VBAC (vaginal birth after Cesarean). Likewise, an extensive evaluation and consultation can be done to review treatments for uterine fibroids or determine whether menopausal patients are candidates for bioidentical hormone therapy.

Ovarian Cancer
It turns out that the patient’s mother had ovarian cancer, and her sister and two maternal aunts were diagnosed with breast cancer – all in the past decade. None of the women had ever been tested to see if they carry a gene that predisposes them to certain types of cancers. What’s more, concerns over radiation exposure had prevented my patient from having mammograms for the past few years, despite the relatively low risk (about the same you would be exposed to on a round trip cross country flight). After I discussed the potential benefits of genetic testing, my medical assistant handed the patient a test kit to obtain a quick saliva sample, which was mailed to a laboratory for testing.
Cervical Cancer
Every year millions of American women find out their Pap smear has come back “abnormal.” After the initial shock and fear, the most common reaction is to ask, “What does this mean?” The good news is that there is no need to panic. pap smear, testingPap test results are abnormal more than 5% of the time—and if you ask your friends, many of them may have dealt with this problem before. Even though the Pap test is a tool for screening cells from your cervix for signs of "precancerous" changes, it is unlikely this will turn into a more serious problem. In fact, precancerous cells known as dysplasia often disappear without any treatment, and only a small number of cases ever result in cervical cancer. If treatment is necessary, doctors now can manage abnormalities less aggressively and less invasively most of the time.
Colon Cancer
Proponents of HRT point to the possible benefits of the therapy: relief of menopausal symptoms including hot flashes, mental clouding, sleep disruption, vaginal dryness and mood alterations, which that have resulted mainly from declining estrogen levels. Other documented benefits include possible improvements in bone density and a reduced risk of colon cancer.

When a patient of mine named Deborah gave birth in December 2008, what should have been a joyous experience soon turned into a nightmare. Although her son was healthy, she began worrying excessively that something bad was going to happen to her newborn after she left the hospital. Worse yet, her thoughts centered on intentionally harming him. Her symptoms were the telltale signs of a severe postpartum mood disorder. Because Deborah had several people involved with her care, including a doula and a supportive husband, her symptoms did not go unnoticed. She was referred to a psychiatrist who specializes in perinatal mental health issues to get through this difficult time. “Despite the best medical care and an amazing husband and community of family and friends, I still suffered tremendously,” she recalls.

It is crucial to recognize the warning signs and seek help as soon as possible. As symptoms become more severe, the risks to both mother and baby begin to rise. The feelings can deepen to the point of having irrational thoughts, delusions, and even psychotic episodes where reality is distorted. The symptoms may linger and become chronic in some women. According to a January 2014 study published in the Harvard Review of Psychiatry, postpartum depression remains a long-term problem for 30 to 50 percent of women. Likewise, a 2013 study of 10,000 women conducted at Northwestern University in Chicago found that 14 percent of the women screened positive for depression. Of that group, nearly 20 percent said that they thought of harming themselves.

MRI
Your doctor can use breast cancer risk assessment tools designed by scientists at the National Cancer Institute to further define your risk of breast cancer in order to help decide on strategies for prevention. If you are at increased risk, you may want to consider estrogen metabolic testing. While a mammogram or MRI pinpoints specific breast lesions or abnormalities, estrogen metabolic testing looks at how your body breaks down estrogen to determine whether it promotes cancer growth.
Mammography
Five years later, most, if not all, insurance plans (except Medicare) still cover annual mammography screening, and most major medical societies like ACOG still support mammograms for women beginning at age 40 (and so do we!). Yet only about two-thirds of women age 40 or older have had a mammogram in the past two years, according to the CDC.
Chemotherapy
But if you do test positive it could be a lifesaver. Women whose test results indicate that they have a BRCA1 or BRCA2 mutation have several options, including stepped up screening, prophylactic chemotherapy, chemoprevention and/or preventive surgeries such as mastectomy and removal of the ovaries and tubes (salpingo-oophorectomy). Integrative approaches including lifestyle changes, which can drastically reduce the risk of developing breast and ovarian cancer -- or at the very least, detect it earlier, are also useful.

Minimally Invasive Surgery
When I first read this, I thought, Really, what constitutes enough? I can recall dozens of abnormalities discovered at an early stage in my patients which, if gone unnoticed, could have become much more significant over time. Examples include STD’s, which might have impacted future fertility, fibroids, which might not have been able to be treated with minimally invasive surgery if they grew larger, and abnormal bleeding related to precancerous cells, which might have become cancer if not treated early. While the ACP might assume that these women would seek care for these conditions, the truth is that many people tend to ignore such symptoms.
Vasectomy
Finally, an alternative is available to tubal ligation for women and vasectomy for men! Robotic Surgery
To clarify, robotic surgery involves the revolutionary new concept of using a surgeon’s own limbs to control the robot’s movements: The physician sits at a console next to a patient, and looks at a three dimensional 10x magnified image of the surgical field. The surgeon’s fingers are attached to controllers, which allow his or her every movement to be translated to the actions of robotic instruments which have been placed through ports into the patient’s abdomen. The robot allows surgeons to move as they would in an open abdominal procedure (as opposed to the “carnival game” movements used in traditional laparoscopy).
Hysterectomy
Laparoscopic hysterectomy: While hysterectomy has been viewed as a last resort, more patients are deciding to undergo the procedure because in the vast majority of cases it can be performed laparoscopically, which means it no longer requires a large incision or long recovery. Laparoscopic supracervical hysterectomy involves removing the troublesome area of the uterus, the source of the fibroids and bleeding, while leaving the cervix (the lower part of the uterus which enters the back of the vagina). What’s more, many women are surprised to learn that it does not necessarily induce menopause, nor does it necessarily entail removing the ovaries.
Endometrial Ablation
Endometrial ablation: In brief, there are now several convenient and effective endometrial ablation (also known as biothermal ablation)procedures where the source of abnormal bleeding, the lining of the uterus, is cauterized, frozen, microwaved or burned with hot water. After committing an afternoon of your time for the procedure, women typically go home and feel better within a day or so, and they end up having an excellent chance of less bleeding for at least the next several years.
Laparoscopy
* Patients experience less bleeding and pain: The improved view of the surgical field allows even the smallest blood vessels and other vital structures to be seen as never before. As a result, routine procedures are sometimes nearly bloodless, and patients experience less incisional pain compared with those who undergo laparoscopy. In addition, post-operative pain from these small Band-Aid sized holes is less than in traditional laparoscopy because the robotic arm movements put less stress on port sites.

Get your Vitamin D levels tested: If you suffer from seasonal depression and are not sure of your vitamin D level, please ask us or one of your other doctors to run the simple vitamin D test called “25 (OH) D.” Our bodies can readily manufacture Vitamin D when our skin is exposed to the ultraviolet rays from natural sunlight. But living in the northern hemisphere during the winter months, we don’t receive enough sunlight to make adequate vitamin D. Many of us are probably not storing up enough D from summer sun exposure due to either direct avoidance of the sun or heavy use of sun block. Let me be clear: I am not advising skipping sun block! But getting about 15 minutes of morning or late afternoon sun without sun block will certainly help to increase your levels. (For those with a personal or family history of skin cancer, year-round vitamin D supplementation is a safe option.)

The good news of late is that aggressively treating low grade, mild dysplasia lesions is a thing of the past. Instead, we can often simply follow them with Pap smears and repeat colposcopy if necessary to see if the problem goes away on its own. Most of the time it will! Even when mild dysplasia (also known as cervical intraepithelial neoplasia or CIN) does develop, your body will get rid of it on its own over 60% of the time.
