        The number one priority of Rehabilitative Medicine Services at Glendale Adventist Medical Center is to assist patients in returning to daily life and the activities they love. Inpatient services are offered in the hospital's spacious rehabilitative wing, while outpatient services are provided in the 20,000 square-foot Therapy & Wellness Center (at 2560 Colorado Blvd). The rehabilitative team is made up of physician specialists, as well as professional and certified staff nurses, therapists and others who meet regularly to ensure that each patient's progress is carefully planned and closely monitored. Comprehensive rehabilitative services include aquatic physical therapy, audiology, outpatient pediatric services, return-to-work programs, hand therapy, speech therapy, neurological rehabilitation services and sports training as well as general physical and occupational therapy.

        On 9/20/16 I went in for my physical therapy evaluation. I was told that unless I sign the forms stating that I will be responsible should my insurance not pay for anything that i would not be treated. This I know is illegal. The receptionist at the counter was confrontational and created a scene.
        On 10/4/16 I went in and the same thing happened. They made me resign the forms and said that they will not accept them with any note on them. What this office is doing is illegal. I now have to find a new therapist because of their ridiculous business practices. Extremely poor customer service and the receptionists are horrible!
