Labor and Delivery
Answer: General anesthesia, or “going to sleep”, is commonly used, but there are other options which may be used in addition or instead. Regional anesthetic, such as an epidural or spinal, is an attractive alternative. An epidural anesthetic is most commonly known for its frequent use during childbirth. In total joint replacement, it allows the patient to undergo the surgical procedure and then additionally can provide excellent pain relief during the early postoperative phase. Patients who choose epidural anesthetic can then either stay awake, be sedated, or have a general anesthetic in addition. Femoral nerve blocks are also becoming more popular. In this procedure, local anesthetic is injected around the main nerve that provides sensation to the knee, and a pump continues to deliver medicine after surgery to relieve pain. Finally, pain medications can be administered by mouth or by i.v. via a PCA, or “patient controlled analgesia” pump, which allows the patient to push a button and get computer-controlled administration of medication.

Carpal Tunnel Syndrome
Carpal tunnel is a common condition involving swelling around the wrist tendons that puts pressure on nerve and tendons that run to the fingertips. This swelling cuts off blood flow, causing a pain in the hand that can range from “pins and needles” sensations to sharp, shooting pain. Common non-surgical solutions include steroid injections, use of a wrist brace and occupational behavior modifications. If these non-surgical options don’t provide relief, then carpal tunnel surgery can relieve pressure on the nerve causing pain in the hand.

A sprained ankle occurs when you roll or twist your ankle in an awkward way in a misstep or injury. This scenario can result in stretching or tearing of the ligaments or tough bands of fibrous tissue that serve to hold the bones in the ankle in place. These ligaments help to stabilize joints and keep them from moving excessively. When these ligaments are stretched beyond their regular range of motion, the result is an ankle sprain.

Tendonitis and Achilles tendon injuries can cause symptoms such as pain down the back of the leg or near the heel, pain that intensifies with activity, pain after exercising, difficulty flexing the affected foot, swelling with pain that worsens with time, and even bone spurs on the heel bone. Though these symptoms can be alleviated with rest, ice, anti-inflammatory drugs, and physical therapy, sometimes the best option is surgery. Symptoms of this type of injury include pain in the ankle particularly when bearing weight, tenderness to touch, swelling, bruising, instability of the ankle, and a limited range of motion. Sometimes rest and staying off the ankle is enough for it to heal, but if symptoms and pain persist, you’ll need to see a foot specialist. Sprained ankle treatment will depend on the severity of the injury.

A bunion is a bony bump that forms on the joint at the base of the big toe when some of the bones in the foot shift out of their proper place. This condition causes the big toe to pull toward the smaller toes, creating a protrusion of the affected joint. The skin over the bunion can also get red, irritated, and sore, and tight shoes can cause even more pain and issues. Bunions can cause continuous pain or pain that comes and goes and can also cause issues with mobility.

Plantar fasciitis is one of the most common causes of pain in the heel. This particular ailment is caused by the inflammation of a thick band of tissue that runs across the bottom of the foot connecting the heel bone to the bones in the toes. Plantar fasciitis pain and symptoms are generally at their worst when sufferers first get up in the morning and take their first steps of the day. The condition often causes an intense, stabbing pain in the bottom of the foot in the heel area. Symptoms can arise when a person stands after sitting and can also be triggered by long periods of standing.
