At your first appointment, expect to spend a couple of hours at our office. You will undergo a complete evaluation for your vein problem, including an ultrasound examination, and meet with one of our physicians to discuss our assessment of your vein disease in detail. Dr. Reeder or Dr. Darnell will show you where there are problems, and recommend a treatment plan to address your specific vein problems. We will also explain the specific treatment options recommended, and answer your questions. Our goal when you leave the office is for you to have a thorough understanding of your specific problem, and feel comfortable with the care we recommend.
MRI
Superficial thrombophlebitis is usually diagnosed through a physical examination by a physician. If deep vein thrombosis is suspected, a venous duplex ultrasound examination is usually the imaging study of choice. If pulmonary embolism is suspected CT or MRI is often performed. Other tests may be required, including tests to detect a genetic blood clotting abnormality, may be needed and ordered at the discretion of the physician.
Radiation Therapy
Lymphedema is swelling that occurs from the accumulation of fluid in the tissues. Normally, during the circulation of the blood, fluid from the bloodstream leaks into the surrounding tissue. In the tissues, the lymphatic system is an open ended microscopic system of thin walled vessels that collect the fluid and ultimately return the fluid to the bloodstream. Lymphedema occurs when there is an overproduction of the lymph fluid which overwhelms the lymphatic system, or impairment of the lymphatic vessels that remove the lymph fluid, the end result being an abnormal accumulation of fluid in the tissue. Lymphedema often results from venous hypertension, because the increased pressure in the thin-walled veins pushes more fluid out of the veins chronically. For this reason, lymphedema is often incorporated into the realm of vein medicine. However, a number of other disorders produce lymphedema unrelated to vein disease. These include genetic disorders, injury, infection, surgery, radiation therapy, and cancer. In tropical areas of third world countries, filariasis infection produced by a parasite is the most common cause of lymphedema. In the United States and developed countries, obesity is the most common cause of lymphedema.

Vascular Surgery
Michael Darnell, M.D. joined Dr. Reeder in January 2014 in the full-time practice of vein and lymphatic medicine and surgery. Dr. Darnell is certified by the American Board of Venous and Lymphatic Medicine. He also brings his experience of greater than 25 years in general and vascular medicine and surgery to the Reeder Vein Institute, and has treated hundreds of patients with vein disease. Following his surgical residency, he was certified by the American Board of Surgery in 1992, and recently completed his second 10 year recertification in 2013. He has vast clinical experience in peripheral vascular surgery, trauma surgery, and cancer surgery. He consistently ranks high in patient satisfaction and clinical outcomes on independent physician review websites.

Ulcer
One of the most challenging problems with advanced vein disease is the occurrence of a venous ulcer, which is a sore or break in the skin. This is a sign of severe vein disease, often longstanding. The ulcer can be small or in some cases involve a large area of skin. They almost always occur in the lower leg above the ankle, often on the inner side of the leg, though they can be seen on the outer or posterior (back) of the leg. The ulcers are usually associated with an area of surrounding inflammation with redness, swelling, and tenderness of the skin and soft tissue.
Lesions
Of course, not all skin ulcerations are caused by vein disease. Arterial insufficiency secondary to atherosclerosis, autoimmune diseases, vasculitis, and skin cancers can produce similar ulcers. A physician experienced in advanced vein disease can usually recognize this problem with examination alone. Ultrasound will confirm the presence of venous reflux and venous hypertension, the root cause of these skin lesions.
