Macular Degeneration
One of the most common retinal diseases is age-related macular degeneration. This, as the name implies, is an age related change in the macula which is the central part of the retina and the area of the retina that allows for reading, recognizing faces and focusing on objects. Most cases of macular degeneration are mild and require no treatment. It has been shown that the progression from early dry macular degeneration can be abated by a few behavioral changes. Quitting smoking and controlling blood pressure are helpful. Certain types of nutritional supplementation have also been shown to help. At Hirshfield Eye Center, we are proud to serve the Queens area with many different vision correction options, which means providing the best in LASIK care.
Cataracts
A cataract is a clouding of the natural lens of your eye that causes blurring or a decrease in vision. The natural crystalline lens that you were born with gradually darkens in color and clouds over as we age. In some cases it is not noticeable and does not cause significant visual loss. In other cases one may start to lose the ability to read, drive, see faces or see the television. Everyone would develop a cataract if they lived long enough. It is not uncommon for cataracts to develop at a younger age such as in one’s forties or even thirties. Even children can develop cataracts that affect their vision.
Cataract Surgery
The new Alcon Toric lens has the ability to correct astigmatism. Those patients with astigmatism have a much higher chance of having good uncorrected vision, that is without glasses, at distance or near when their astigmatism is corrected with the toric lens. Astigmatism is a condition where the cornea is not completely round like a basketball but somewhat like a football lying on its side. One axis has a different power than the other. The toric lens is very good at accurately correcting this. The drawback of the toric lens is that it is not available as a multifocal lens or accommodating lens so while this lens will give you much better vision without glasses, you will likely need reading glasses. The toric lens, offered at Hirshfield Eye Associates in Queens, is often a good option for patients with astigmatism who also have cataract surgery.
Eye Exam
After an eye exam, contact lenses are fitted by a certified contact lens specialist directly supervised by our ophthalmologists in Queens. We fit all types of common and specialty lenses. We carry all the top brands of contact lenses including Acuvue, Coopervision, Biomedics and Hydrogel as well as less common custom lenses. Daily disposables, biweekly disposables as well as planned replacement lenses are all available in powers that correct farsightedness, nearsightedness and astigmatism. For those of us over 40 years old that require both far and near prescriptions we offer monovision contacts, bifocal contacts and the newest progressive addition contact lenses. For patients that have corneal disease causing irregular corneas and poor vision we have available all types of rigid gas permeable(RGP) and hybrid lenses. RGP lenses will also give the clearest possible vision in patients with high levels of astigmatism.

Multiple Sclerosis
Other neurological conditions include optic nerve disease, muscle palsies in the eye, multiple sclerosis, myasthenia gravis, stroke, brain inflammations and other rare diseases. Many neuro-ophthalmological conditions are self limited and improve with time, requiring no further procedures. Some are true life threatening emergencies. Others, while not life threatening, require immediate and appropriate referrals to the neurologist, neurosurgeon or other subspecialists. The key to success is a quick and correct diagnosis. The only way to make the right diagnosis is by seeing a skilled ophthalmologist who is able to pay close attention to the history and exam, to pick up an often subtle and difficult to diagnose disease. Diabetic Retinopathy
Other common diseases of the retina include diabetic retinopathy which is discussed in the section of diabetes and the eye. Blocked blood vessels can also occur and usually present with acute loss of vision. This sometimes requires treatment with a laser. Retinal tears and detachment may not have symptoms at all but oftentimes present with floaters, flashes and/or loss of vision. Symptomatic retinal tears can usually be treated in the office with a painless laser procedure. Retinal detachment usually requires surgical intervention and, if treated early, can yield very good vision after treatment.

Ultrasound
Cataract surgery is one of the most commonly performed procedures in the United States. It is extremely effective in restoring vision and quality of life. While no procedure can be guaranteed, the success rate of cataract surgery is at least 98 %. The time from arrival to our surgical center in Queens to your trip home is often less than 2 hours. It is a painless procedure usually performed in 15 minutes or less and is done under topical or local anesthesia. Microincisions are used and are less than 3 mm and therefore require no suturing. The cataract is emulsified in the eye by an ultrasound probe or a tiny water jet that allows us to use these small incisions. After the cataract is removed an intraocular lens implant is placed in the eye so that the vision is restored. It is very unusual to have symptoms worse than a mild irritation or foreign body sensation post operatively. In most cases resuming of normal activities will be the same day as the cataract surgery or the next day. The vision can become much clearer by the first day.

Eyelid Surgery
Many people, both young and old complain of puffy eyelids giving them a tired appearance. This is a natural result of a combination of family background and aging. Cosmetic eyelid surgery can relieve this problem making a patient look younger. Botox injections can relieve forehead furrows and frown lines as well as crow’s feet around the eyelids. The bottom line is that very close attention to a patient’s symptoms combined with a very detailed examination can usually lead to the correct diagnosis and intervention to relieve some very disturbing symptoms. Skin Cancer
Some oculoplastic problems can be treated by procedures here in the office of Hirshfield Eye Associates in Queens without surgery. One of the more common problems is trichiasis which is a condition where lashes grow in a direction that scratch the surface of the eye. These can be treated by simply removing them with a forceps or with electrolysis. Many oculoplastic conditions do require surgery. These include conditions such as the eyelids turning in (entropion) or out (ectropion). Droopy upper eyelids causing loss of vision can be raised surgically. The eyelids are a very common location for skin cancer, especially basal cell carcinoma, and these can often be cured by surgically removing them. A common complaint is tearing and this sometimes is a result of a blocked tear duct and if so, surgical correction procedure is usually required and effective.
